This blue tearstone from Catarina boosts the
defence of its wearer when in danger.

Undead Burg

From the Undead Burg bonfire, cross the bridge under firebombs and continue through the buildings toward the rolling-barrel stairs and the tower leading to Taurus Demon.
Before climbing toward the tower, take the side stairs down into the narrow passage occupied by the sword-wielding Black Knight.
Defeat the knight or draw him away, then collect the ring from the corpse at the dead end behind his starting position.

Notes

Low-health defense: Raises physical, magic, fire and lightning defense by 50% while your HP is below 20% of its current maximum. This is a defense-stat increase, not a flat 50% reduction to every incoming hit.
Activation: The bonus needs to be active before an attack lands. A hit that takes you from above the threshold to death is not rescued by the ring.
Health changes: Healing out of the low-health range switches the effect off. Equipment that changes maximum HP also changes the amount of HP at which the ring becomes active.
